Departure Flight Request

Please complete and return this flight request so that we can schedule your flight. If you choose to use your 30-day grace period, we are happy to schedule your flight from the airport that you will be near at the end of your travels. When planning your final weeks in the United States, it is important that you keep in mind we do not guarantee a flight on a specific date and require a five-day window. When providing departure dates, please provide a five-day window (for example: July 11 - 15).
We do not book flights more than 29 days into your grace period, in case your international flight is cancelled, and you are forced to stay in the US for an additional day.
We can only guarantee departure and arrival airports included on our list of permissible airports, in the drop-down box. You may note a preferred departure and/or arrival airport, and we will try our best to accommodate your wishes if at all possible.
You must complete this form at least 30 days in advance of your preferred return date. Generally, we will book flights no more than 60 days before your departure date. Once your flight is booked, we are unable to make changes to the flight. If circumstances absolutely require a change in flight plans, you are responsible for any change fees incurred and/or additional cost.
By completing the form below, you accept that the value of the ticket is to be refunded to Expert AuPair if you decide not to leave the United States during your grace period. If you do not take your flight, then you must tell us before the flight leaves. Failure to do so is against program rules and can result in program termination.
Your health insurance coverage ends on your last working day. Please note that if you choose to use your travel month (grace period) the cost of your insurance is $90.00 for that month. Let us know if you are choosing this option.
